- First Letter of the Last Name is: A or B = benefits available on the 1st
- First Letter of the Last Name is: C = benefits available on the 2nd
- First Letter of the Last Name is: D, E, or F = benefits available on the 3rd
- First Letter of the Last Name is: G or H = benefits available on the 4th
- First Letter of the Last Name is: I, J, or K = benefits available on the 5th
